Click on the appropriate file to view the claim form, print out the claim form. All questions must be completed fully and honestly, a dishonest answer may result in you having no insurance cover. If you are unsure of any question, contact this Office for assistance. The claim form must be signed and dated by the policyholder. Post the completed claim form and any additional information to our Office.
